At Story Dental, we believe in making dentistry as convenient as possible. Providing online patient forms is one way we are accomplishing this by gathering detailed information from our patients. You will no longer need to wait to update and fill out your information at your appointment visit.  Just print and bring your forms with you to your appointment! We provide both English and Spanish forms for your convenience.
